PeerPlace® Connects EMTs and Transitional Case Managers for GVHP
Genesee Valley Health Partnership, the rural health partnership in Livingston County, NY, is one of the newest PeerPlace® customers. They are the provider for NY Connects in that county as well as the Livingston Help for Seniors Program. This is a unique program where county EMTs have been trained to do a short screening before transporting older adults by ambulance to Strong Memorial Hospital. At discharge , a transitional case manager assesses their needs and determines whether they should be referred (through PeerPlace®, of course!) to the Office for the Aging for follow-up services. An unexpected benefit: EMTs are reporting higher satisfaction with their jobs knowing these elders will not "fall through the cracks!"
