New York Medicaid Redesign Teams Health Information Technology Initiative selects PeerPlace Automated Eligibility Screening and Universal Referral System to enhance the coordination of care for Medicaid-eligible pregnant women.

Alabama AAAs rollout Square One, a decision control system powered by PeerPlace. Square One ensures a standard level of service across an organization by automating the assessment of client needs and matching the identified need with eligible Federal, State and Local programs. Square One is a natural extension of the PeerPlace "No Wrong Door" ADRC Model supporting seniors and adults with disabilities to ensure the client is receiving the right benefits and recommendations of eligible services.

Albany DOH contracts with PeerPlace to deploy a No Wrong Door, single point of entry, automated eligibility screening and referral system.

PeerPlace delivers Alabama's Medicaid Universal Intake Form (UIF) for all AAAs in Alabama.

PeerPlace contracts with the Aging New York Foundation of New York City to provide Mental Health Client Tracking, Case Management and mental health assessment tools to track and support Super Storm Sandy Victims.

Merced County California Selects PeerPlace to track and manage Prevention and Early Intervention Programs



CARE TRANSITIONS
 
Virginia Department for Aging and Rehabilitative Services partners with PeerPlace to deploy a Care Transitions Module that is integrated with the statewide NAPIS Client Tracking System, ADRC Resource Directory and the Communication, Referral, Information and Assistance program.
