PeerPlace Information Exchange (PPIX)

PPIX is built on MIRTH Connect® that allows the secure exchange of data following national standards and common data sets required to collaborate with the following types of systems:

  • Community-based information systems
  • Clinical information systems (e.g. EHRs)
  • State Medicaid systems and/or other payer systems
  • Health Information Exchange systems
  • Personal health records

We continue to incorporate national data standards from the Office of National Coordinator as they become available. PeerPlace is in alignment with the ONC S&I Framework and is a member of the eLTSS Standards committee.

PeerPlace Information Exchange (PPIX) LTSS

PPIX has maintained an HL7 compliant interface with the Rochester RHIO since 2009. PPIX delivers accurate and timely home- and community-based service data available at the point of care. PPIX renders community care summary data through the HIE's Virtual Health Record (VHR) to over 3,800 healthcare providers in a 10 county region. The Rochester RHIO follows HIPAA and Patient Consent Standards.

Care Management (PeerPlace CM) Perinatal

PeerPlace CM, including PPIX, was selected by the New York State Medicaid Redesign Team (MRT) for a Health Information Technology (HIT) project to enhance the coordination of care for Medicaid-eligible pregnant women across four installations; Rochester, Syracuse, Lower Hudson Valley and Brooklyn. The interoperable landscape consisted of existing Health Information Exchanges, PeerPlace Eligibility Screening and Universal Referral Module, PeerPlace Case Management system, and the existing relationships among the medical systems, the health plans and perinatal support services. The goal is to share both medical and psychosocial information efficiently with all the providers who form the system of care for a particular patient.

Connecting Customers to Providers

PeerPlace provides a Client Level Service Unit (CLSU), an interface that leverages PPIX to import program service level data into PeerPlace from other external systems and service providers. This allows required consumer records and service order information to be shared with state and local contracting agencies without having to enter data in two systems.
