Key Initiatives

PeerPlace is aligned with the Administration on Community Living (ACL) and Centers for Medicare & Medicaid Services (CMS) LTSS strategic initiatives and programs. PeerPlace offers a scalable data management software solution to support our clients in implementing and managing these initiatives.

PeerPlace ADRC Software

PeerPlace ADRC provides the tools and services required to assist states with transforming and/or consolidating their long-term services and supports (LTSS) into a single No Wrong Door (NWD) system for all populations and all payers.

The ADRC platform from PeerPlace provides streamlined access to public and private long-term services and supports, including information on Medicaid home- and community-based services as well as other benefits and programs. Our platform establishes one statewide resource directory to deliver consistent information to the public, information and referral (I&R) specialists and case managers and supports individuals seeking public and/or private resources to assist with the planning for their future long-term care needs.

Information & Referral

The PeerPlace Information and Referral module (PeerPlace I&R) is a single-point-of-entry system designed to link call center information and assistance specialist with LTSS resources to deliver consistent information to consumers.

PeerPlace I&R delivers an interface that allows I&R specialists to track, data enter, search for resources on eligible services and manage multiple information requests at one time while seamlessly communicating with the consumer. This module offers reports and work lists that provide real-time visibility into open and completed cases and seamlessly collects required NAPIS reporting data throughout each engagement.

Automated Eligibility & Enrollment

LTSS - PeerPlace provides a decision control software solution called Square One. Square One ensures a standard level of service by automating the assessment of client needs and matching the need with eligible federal, state and local programs. This system is a natural extension of the PeerPlace "No Wrong Door" ADRC Model.

Maternal, Infant and Child - Our Cloud-based data management software supports centralized risk assessments, referrals and coordinated follow-up services across multiple participating health and human service providers. The system provides ongoing reinforcement, follow up and systems navigation support of high-need patients—integrating service delivery and all providers.

Case Management

PeerPlace Case Management module (PeerPlace CM) provides a case management platform to allow agencies, providers, professionals, consumers and family members to manage securely the delivery of multiple home and community-based services. The module is utilized effectively by state and local agencies and service providers to manage CMS, ACL, grant and levy-funded LTSS programs.

PeerPlace CM manages the entire lifecycle of case management from proving initial information and referrals to intake and assessment, case management and coordinated care planning and reporting across multiple services, funding sources and providers.

Multi-System Interoperability

PeerPlace Information Exchange (PPIX) is built upon MIRTH Connect® that enables inter-disciplinary care teams to achieve interoperability and manage collaboratively LTSS for their members and patients. PPIX allows users to exchange data in accordance with national standards and common data sets. The module integrates with the following types of systems:

  • Community based information systems
  • Clinical Information Systems (e.g. EHRs)
  • State Medicaid Systems and / or other Payer Systems
  • Health Information Exchange Systems
  • Personal Health Records

Balancing Incentive Program (BIP)

The PeerPlace Data Management System provides an interoperable platform to achieve a statewide "No Wrong Door" Single Point of Entry System. Consumers and caregivers are able to access Long Term Services and Supports 24/7.

Our system is designed to (manage securely) multiple programs within a single master database while sharing standardized assessments.

Business Intelligence (BI) tools are available to track and report on data collected throughout the lifecycle of consumer management, including initial information gathering and consultation, eligibility determination, service plan authorization, service delivery and billing.

Universal Assessment

PeerPlace has been approved by interRAI to integrate and deliver inter-rater reliable screening and assessment tools across multiple populations, which meet BIP requirements for standardized assessment tools. This delivers true analytical reporting and trending of evidence-based outcomes and longitudinal analysis.
